2. Have you ever tested positive for the Human Immunodeficiency Virus (HIV) or do you have Acquired Immune Deficiency Syndrome or any another AIDS-related illness?
3. Do you have an incurable heart condition leading to a life expectancy of less than 24 months?
4. Over the past three years, have you been diagnosed or treated for any form of cancer, suffered an angina attack or myocardial infarction, undergone cardiac surgery, had a stroke, received an organ donation or bone marrow, or been advised that your condition required such a procedure?
